Follow-Up Appointments: $150
Cancellation Policy
We ask that you provide at least 24 hours' notice if you need to cancel or reschedule an appointment. Appointments canceled with less than 24 hours' notice or missed without notice may be subject to a cancellation fee of up to $150.
Medicaid & Medicare
We are not enrolled to bill Medicare or Medicaid for services provided. Patients who choose to receive care at our practice are responsible for payment at the time of service. Claims will not be submitted to Medicare or Medicaid on the patient's behalf. Before establishing care, patients with Medicare or Medicaid coverage will be asked to sign a Financial Responsibility Acknowledgment confirming their understanding that they are personally responsible for all charges incurred.
